- Annual Exams and Preventive Care – including certain diagnostic screenings such as prostate and cervical cancer.
- Outpatient Labs and Radiology Services – diagnostic imaging and scans including X-rays, EKG, bloodwork, MRI, CT and PET scan.
- Colonoscopies and Mammograms – diagnostic and preventative screenings for fire fighters, spouses or dependents, regardless of age or frequency.
- Dental Diagnostic and Preventive Services – Includes exams, cleaning, fluoride, x-rays, and sealants.
- Virtual Care through Teledoc including behavioral health services.
- Vision Exam covered in full – vision hardware 100% covered up to $600 every 2 calendar years.

Your copay helps you with the following visits:
- Unlimited Outpatient Visits for behavioral health visits (counseling) including mental health or substance abuse services.
- Spinal Manipulation/Chiropractic care (24 visits PCY)
- Acupuncture (24 visits PCY)
- Outpatient Rehabilitation Therapy such as physical, speech, occupational and massage therapy services (45 PCY)

First,
Once you’re logged in, click Investment Performance & Research, under your account balance.
To make changes click on the Make Changes tab near the top of the page.
